Pathology
The patella slides up and down in the femur groove when the knee joint is extended. If you frequently perform weight-bearing knee bending exercises for a long period, or if the patella is misaligned, the cartilage under the patella may be overloaded and worn.
Causes
Repeated long weight-bearing movements of the knee
Standing in high heels for a long time will cause the knee joint to hyperextend
Tight quadriceps, insufficient strength, and imbalance of inner and outer muscle strength
Trauma
Poor gluteal muscle strength
Symptoms
There is a grinding sound or feeling when the knee joint is bent
Pain worsens when going up or down stairs or bending the knee
In severe cases, the knee joint may become swollen and stiff
Knees are sore and weak during weight-bearing movements
Treatments

Consequences/Grading of Not Treating Early
Level 1: Local cartilage softens and color changes slightly
Level 2: The local cartilage surface is slightly uneven, with some small cracks or fibrosis.
Level 3: The cracks on the cartilage surface extend deep into the hard bone beneath the cartilage, and the cartilage forms a crab-like surface.
Level 4: The cartilage is gone and the subchondral hard bone is exposed and worn away
